For duvet covers, use the dimensions of your duvet, not your mattress. For muslin blankets, bedspreads and throws, measure the width of your bed and allow for the desired drop on both sides. Example: a 180 cm-wide blanket on a 160 cm-wide bed will drop about 10 cm on each side. For a sofa, measure the area you want to cover and choose whether you prefer the blanket loosely draped or covering more of the seat and armrests.
